Reporting Forms for Independent Schools

Reports

The following reports are required from all member schools at regular intervals by the Commission on Independent Schools.

Two-Year Progress Report

The Commission on Independent Schools requires that you submit a Two-Year Progress Report due on the date noted in the letter received by the school following Commission action on the evaluation visit. Schools will be emailed a reminder letter with form attached prior to the report due date. Please email your report and supporting documents to: cisreports@neasc.org

Five-Year Progress Report

Other Information/Reports

Additional reports may be requested from time to time. Please note that a Special Progress Report does not follow a set template and specific information will be requested upon assignment of the report.

Merged Schools Report

A school will submit a Report for Merged Schools when two or more schools merge. Please report a school merger by sending us a letter or reporting it on your Annual Report in the fall. You may also contact a member of our staff for more information, including specific due dates.

If a report for merged schools has been requested, please submit an electronic copy of the completed report to cisreports@neasc.org. Upon receipt of the report, NEASC staff will determine whether a visit to the school is warranted to supplement and clarify the information provided.

Substantive Change Report

Examples of substantive changes include adopting a new mission, purpose, or admitting different students (e.g., changing from a traditional school to a therapeutic school), adding a division (e.g., elementary school, middle school, high school, post-graduate), adding a residential or homestay program, adding or removing enrollment of a sex (e.g., single-sex to coed), adding a new local campus that will serve current students, and changing ownership for a proprietary school. The merging of two or more schools is also a substantive change but is addressed under the Merged School Policy.

Please report substantive changes by sending us a letter or reporting it on your Annual Report in the fall. You may also contact a member of our staff for more information, including specific due dates.
